  • Persons who struggle with emotional difficulties, mental health illnesses, relationship problems, addictions, substance use disorders or problems in everyday living
  • Persons who need a family doctor
  • Uninsured (Sliding Fees based on Financial Assessment)
  • Persons with private insurance, Medicaid or Medicare

    POLK WELLNESS CENTER PROVIDES

  • Primary medical care
  • Assessment and treatment; individual/group therapy for mental health, substance use disorders and co-occurring disorders
  • Assessment and treatment for phase-of-life problems, relationship problems and periods of emotional distress, such as depression and anxiety
  • Consultation to community agencies
  • Psychological, psychiatric and medical evaluations
  • Prevention, education and wellness services that promote a healthy lifestyle
  • An integrated model that addresses caring for the mind and body

CRISIS

  • If you are in crisis and require EMERGENCY TREATMENT, please call 9-1-1 immediately or seek treatment at the nearest hospital Emergency Room.
  • Individuals who may be suffering from a crisis or who are dealing with acute emotional needs--and are not in an emergency situation--will receive an assessment from a highly experienced member of the Polk Wellness Center staff.
  • Clients in crisis will receive a brief medical screening, supportive care, and if needed, see a therapist for an evaluation.
