The Family Medicine Clinic will be moving next door to the Mental Health Clinic and Administrative Offices effective February 18, 2013.

Polk Wellness Center offers comprehensive health and wellness services to Polk County and surrounding areas. The Center uses an integrated model to provide primary medical care, mental health/substance abuse counseling, illlness prevention, education, wellness, and recovery support services.
