Polk Wellness Center Adds Diabetes Education Self Management and Wellness Program in collaboration with St. Luke's Hospital

 


Melissa Melum, RN, CDE (Certified Diabetes Educator) has worked with patients across the spectrum of diabetes, from newly diagnosed to patients on insulin pumps, at Kaiser Permanente (Gaithersburg, MD) and the Joslin Diabetes Center affilliate at University of Maryland/Maryland General Hospital.

The Diabetes Self-Management and Wellness Program at Polk Wellness Center provides a comprehensive approach in which the person with diabetes learns how to manage all aspects of the condition; from monitoring, medications and diet to psychological stress, nutrition and lifestyle change.  The program is open to all residents with a referral from their primary physician.

211 Staff Visits Polk Wellness Center

 

 

Polk Wellness is a participant in the Polk County 211 Program, a free, confidential, 24/7 Help Line.  Staff members of 211 had a 'whistle stop' tour of Polk County agencies in resource listings on October 13.

Fundraiser to Support Polk Wellness Center

The First Inaugural Loud Pants Open

The 'Loud Pants Open' event will take place on September 13, 2014 at the Links O' Tryon Golf Course.  Our goal is to raise money and awareness that will benefit Polk Wellness Center as they provide medical and mental health services to residents of our comunity and the surrounding area.  Polk Wellness Center serves the needs of Polk county and surrounding area.  Proceeds from this event will support services for uninsured.  We are grateful to Brenda Drake and Mike Asheworth for organizing this event.  Detailed information may be found at www.loudpantsopen.com.
